　编者按：北京同仁医院医生被砍、哈医大实习医生身亡、北大人民医院医生被刺，近期，医患关系又成社会焦点。由著名作家、《蜗居》和《双面胶》的作者六六担任编剧的医疗剧《心术》也将于近期开播。曾以患者和医生的身份“卧底”医院半年的六六，如何看待当前的医患关系？健康时报记者专访六六，试图为观察医患关系提供一个第三方视角。

　　《心术》出版已有三年，我发现，这三年来，至少从表面上看，我们的医患关系并没有缓解，反而是尖锐了。

　　同仁医院医生被砍、哈医大实习医生身亡、北大人民医院医生被刺，最近的新闻，看了让人心寒。

　　这段时间，我的信箱都快成了医生们的知心大姐姐信箱，好多医生给我发邮件发微博私信，说他们的困惑、痛苦和想要撤退的决心。

　　所有患者遇到的问题，我都遇到过

　　写《心术》，缘起是2007年母亲因乳腺癌住院。

　　所有患者遇到的问题，在这个过程中，我都遇到过。总觉得医护不够贴心，担心他们给多用药……甚至手术后很久，母亲经常胳膊疼，还一度怀疑这个手术是否真的必要。

　　跟医生讨论后，我惊奇地发现，不同的医生给出了完全不同的见解。有说，乳房切除是对的，因为母亲这个年纪，生命高于一切，割除最安全有效。也有说，人活着，生活质量很重要，母亲属乳腺癌早期，剔除病灶即可，没必要切除。

　　这两派医生，都是我的朋友。

　　于是我理解了，同一种病症，原来其实并没有一个标准的治疗方案，抛开利益之外，医生也会基于个人对医学的认知水平给出不同的判断。

　　有意思的是，我还发现，与偶尔才生病去医院的人不一样，像母亲这种长期的病患，其实跟医生的关系都非常良好。因为时间久了，我们发现，医生并不冷漠，而是因为治病救人，必须相信自己的判断、必须承担生死的选择，所以“必须孤傲”。

　　很多人对医生的误解，最直接的原因就是医生的“孤傲”，总觉得医生态度不好脸难看，医生呢，一上午要看几十个病人，喝水上厕所都没时间，三两句话也没法安抚患者，矛盾由此产生。

　　相互对立久了，彼此不信任

　　我一个朋友，最近有感冒症状五六天都不好，跑到医院去看，医生诊断就是感冒，让他回家了。结果，引发脑膜炎，几近死亡的边缘。

　　举这个例子，是想说，医学是很复杂的，不要总抱有“一个小感冒怎么就要看掉三五百块钱”的成见，质疑医生开出的各种化验单，一些与感冒不相关的检查，其实是在排查与感冒症状相似的其他疾病。

　　医生和病患之间发生冲突，绝大多数来源于信息不对称。患者不懂，医生又没时间解释，矛盾丛生。

　　其实，医患信息不对称，在全世界都存在。但在其他国家，医生都是有绝对权威的，只有在中国，医生缺乏权威，他会问你你想怎么治。

　　为什么？因为相互对立久了，彼此不信任。

　　2009年我摔断胳膊，医生告诉我有两种治疗方案，一种是保守治疗，让骨头长在胳膊里，缺点是阴天下雨可能会疼。另一个是开刀治疗，取出碎片，缺点是有可能损伤运动神经。

　　正常情况下，医生会说：“我告诉你两种后果，你自己决定治疗方法，我不能替你做决断。”

　　我信任他，让他替我决断，他最终让我不开刀保守治疗。我的信任，为我省了一刀的费用和疼痛，且保护了我的运动神经。目前为止没出现下雨疼痛的症状。

　　社会病了，医患在吃药

　　不信任，不能怪患者。整个社会都存在这样一种不信任，医患关系只是社会矛盾冲突的焦点，是社会病了，医患在吃药。因为它跟生死相关，爆发得更强烈。

　　如果我们有完善的医保制度，药品公平竞争，国家提高医生待遇，那么我们或许就不会再把原本属于制度的问题，直接推及到人与人之间的诚信上去。但体制保障不可能一蹴而就，我们还是要给制度一点耐心。

　　不要总是把手指头指向别人，责问别人为我们做了什么，也要看看，我为社会做了什么。

　　对那些萌生退意的医生，我唯一能做的，就是安慰他们、鼓励他们，告诉他们绝大多数的患者跟他们一样也是好人、是善良的人，不要因为那10%或者5%的患者丧失了对整个社会的信心。

　　这也是我要给老百姓说的，绝大多数的医生是善良的。如果你相信自己是好人，你就要相信，坐在你对面的另一个人也是好人，因为只有信任，才能让社会改变。

　　医护不仅看病，还要看心

　　人在生病的时候，是最悲观失望对一切产生怀疑的时候。重新建立起一种良性循环，一定需要有一方先伸出手来。

　　我们不能对患者要求更多。我一直强调，患者是弱势群体。无论多强的人，只要一生病，他就是弱者，医护人员承载的任务不仅是看病，还有看心。

　　“卧底”医院期间，我对此深有体会。

　　穿着白大褂，挂着照片和名牌，我这身“医生”的打扮外加一副和善的面孔迷惑了不少病急乱投医、抓住一个医生就不停倾诉的患者。我抽出相当一部分时间听他们说疾病的起源，家庭的纠纷，老板的残酷，未来的担忧，甚至房贷还有多少没还等等，听到动情处，我会提出我的意见甚至给予拥抱。

　　让我诧异的是，完全没有医学背景的我，却起到了甚至连医生都起不到的作用。在我离开医院以后的好几个月，还有病人跟人打听，说“这个大夫，人特好”。

　　所以我尽量地告诉医生朋友们，你如果换一种柔和的语气，多说哪怕一句关爱的话，病患和家属，都对你恨不起来。
